Custom Questionnaire Setup
Deliver one questionnaire based on CDC’s COVID-19 self-checker or customized questionnaire template provided by client POC. Delivery times vary from 3 to 5 days based on the complexity of each questionnaire.
Deliver database access to Client POC. Through the database, POC can view and export the following questionnaire data: name, date/time stamp and survey outcome.
Please note: LiveSafe’s system for handling Customer Data is stored in a HIPAA-compliant database. Customer is solely responsible for complying with all applicable laws and regulations concerning its use of LiveSafe and its LiveSafe Customer Data, including content and forms creation and requesting and handling information responses.
Following initial delivery, changes to your questionnaire will incur a $250 change fee.
